Introduction
Congenital situs inversus totalis with dextrocardia is a rare condition. This case was
incidentally diagnosed in Guyana in a patient presenting with hyponatremia and uncontrolled
diabetes mellitus.
Case
by the Case: The importance of physical examination and imaging in
diagnosing dextrocardia with situs inversus.
Case Pr esentation: A 65-year -old female with a history of hypertension and diabetes mellitus, non-
compliant with medications, presented with polyuria, dysuria, lower abdominal pain, vomiting, and
lethar gy. Laboratory findings revealed mild diabetic ketoacidosis and hyponatremia. No cardiac
complaints were noted. Physical examination revealed distant apical heart sounds on the left and
prominence on the right hemithorax. Further investigations with electrocardiogram, chest X-ray ,
and abdominal ultrasound led to the incidental diagnosis of dextrocardia with situs inversus.
Conclusion
Dextrocardia can be suspected based on physical examination alone and confirmed
using radiographic modalities, leading to better outcomes for patients diagnosed with this rare
congenital condition.
